(At the time of posting there appears to be a similar issue whereby cmd+w doesn't close a tab when it's a single tab, but this issue varies somewhat. It is also somewhat erratic and difficult to replicate.)
Steps to replicate:
(1) Open a series of tabs
(2) Browse for a while
(3) Try closing tabs using shortcut key cmd +w.
Expected behaviour:
Each tab in focus/selected should close.
Actual behaviour:
The 'file' menu in the menu bar briefly flashes an active highlight, but the tab is never closed.
(I've been unable to replicate this issue on demand, but have experienced it. I don't believe in the supernatural.)
