When I press the Refresh button in the URL bar, nothing happens. Selecting the URL and pressing Enter causes the page to Refresh; so does View -> Reload this Page; so does Cmd-R. But the button seems to be disconnected.
I don't have a recipe per se -- this is completely consistent for me.
Don't know how long this has been the case: I noticed the problem a few days ago, but it might have started well before then.

Not normal behavior, so we'd need steps to reproduce the state.
Not sure what to say here -- in exploring more tabs, it's not always happening, but is doing so much of the time for me. (Including on this page.)
Interesting: in exploring more, it's intermittent -- in the past ten minutes, I've observed at least a couple of pages where it succeeded the first time, but failed subsequently. (Eg, that just happened for a couple of GDocs I have open.) Possibly it's a state that the browser is getting into, although there's no obvious cause -- after a couple of successful reloads, now the button is consistently unresponsive for me again.
Have had same experience with Orion for a long time. For me its not only refresh not working sometimes, its also X button to stop a process if it gets stuck
