Yes, it looks like two conditions are being triggered in the case I show above above: Post in a follwed discussion, and a reply. If I get a reply in a followed discussion, I get two emails, and it might make sense to just get one instead.
A separate functionality on the same topic: it could be nice to just "pool" these notifications for a minute per a given discussion thread and then dispatch them all in a single email. A re-flag, re-name, reply etc. often happen at the same time within a short time-span.
